We appreciate your interest in applying for admission to Eger Health Care and Rehabilitation Center. An application and procedures for admission are available as well as additional information about specific programs and services.  During the recent resurgence of COVID, the facility has postponed tours but please call Admissions for further information.

We take pride in our reputation for excellence which we have earned since our founding in 1916. We are affiliated with the Evangelical Lutheran Church in America’s Division of Social Ministry with a specific mission to bring comfort, healing and hospitality to all persons, especially the aged and infirm.

Eger Health Care and Rehabilitation Center is a 378 bed residential health care facility which provides twenty-four (24) hour skilled nursing care. We also offer short-term rehabilitation, sub-acute, complex medical and nursing care, palliative and hospice care.

Payment for the care provided at Eger  may be covered by Medicare, an individual insurance policy, private resources or Medicaid, depending on  the prospective resident’s condition and circumstances. An application will be prepared for Medicaid coverage, if a resident meets the resource levels.

The Eger application for admission  must be completed by the applicant or family member. If the applicant is in the hospital, the hospital will send Eger a  Patient Review Instrument (PRI) and Screen. If the applicant is at home, a certified home health agency can send a nurse to the home to conduct the PRI evaluation.
